Exercise Snacks Enabled by an mHealth Intervention Delivered in General Practice to Promote a Sustainable Active Lifestyle in Adults with Obesity

AnotationObesity is a growing public health challenge, contributing to chronic conditions and increasing healthcare burdens. While physical activity (PA) is essential for obesity management, many struggle to sustain regular PA due to physiological and psychological barriers. This project evaluates an mHealth-supported PA intervention centered on exercise snacks to promote sustained activity in adults with obesity. Integrated into primary care, it combines supervised structured exercise, individualized counselling, and just-in-time adaptive interventions (JITAIs) via wearable activity trackers and the HealthReact platform. At the core is an 18-month randomized controlled trial (RCT) with 306 participants from general practices. The intervention includes a six-month intensive phase with supervised exercise and human support, followed by a 12-month maintenance phase with fully automated PA support. The control group receives standard lifestyle recommendations. Co-primary outcomes are changes in moderate-to-vigorous PA (MVPA) via accelerometry and aerobic capacity (VO2 max) from graded exercise testing. Secondary outcomes include cardiometabolic biomarkers, physical function, patient-reported outcomes, and body composition. Beyond the RCT, a process evaluation will assess adherence, engagement with mHealth tools, and participant experiences. An embedded Ecological Momentary Assessment (EMA) study will explore psychological and contextual determinants of PA to optimize JITAI delivery. Building on prior research in PA interventions and exercise snacks, this project leverages digital health and behavioral strategies to promote long-term PA adherence. By integrating PA support into primary care, it provides a scalable, sustainable approach to improving PA in adults with obesity, reducing health risks, and enhancing well-being.
